Inflammatory bowel disease


The CME during the last IMA meeting held on 26th May 2010, was conducted by the Cochin Gut Club. The theme was Inflammatory Bowel Disease (IBD). This topic was selected because the World Gastroenerology Organisation has named the year 2010 as the “Year of Inflammatory Bowel Disease”. Moreover May 29th was the World Digestive Disease Health Day. The CME was chaired by Dr. K. Prakash, the President of Gut club and GI Surgeon of PVS Memorial Hospital. The speakers were Dr. Varghese Jaison, Consultant Gastroenterologist, Lourdes Hospital, Dr. Geetha Sanil, Pediatric Gasroenterologist, AIMS Hospital, Dr. Charles Panakal, Consultant Gastroenterologist, Medical Trust Hospital and Dr. Byju Kundil, Consultant GI Surgeon, Lakeshore Hospital. Dr Varghese Jaison gave an overview of Inflammatory Bowel Disease. The main diseases which come under the category of IBD are Ulcerative Colitis, Crohn’s disease and Indeterminate Colitis. He discussed on the clinical features, raintestinal manifestations and investigations which need to be carried out in a patient with IBD. He also stressed the points to differentiate IBD from Irritable Bowel Syndrome (IBS). Colonoscopy forms a major investigation to differentiate theses two illness. Dr. Geetha discussed IBD in children. It can be seen from infancy to adolescent age group, even though the disease predominates in the latter. The presentation in children differs, with failure to thrive being a major manifestation. In smaller children the main differential diagnosis is immunodeficiency states. Dr. Charles talked on the overview of IBD management. The main stay of treatment is using Steroids to induce remission and immunomodulators, 5-ASA and biologicals to maintain remission. Various treatment options like Step up and Top down treatment algorithms were also discussed.  Dr. Baiju talked on the indications, types of surgery etc. in IBD. He also discussed about the various pouches after colectomy in ulcerative colitis and its complications.
